The Importance of a Treatment Plan Template in Healthcare Organizations

When treating patients, it is essential for clinicians, doctors and other healthcare workers to come up with a treatment plan. Such a plan would include information regarding the drugs that the patient will take and for what time period. A treatment plan can also include information on the degree at which the patient is ready to change positively. This is important especially when treating patients with behavioral illnesses and this has led some healthcare facilities such as Managed Behavioral Healthcare Organizations (MBHOs) to come up with a treatment plan template that they use as a guide when treating their patients. Such templates usually give information regarding the minimum requirements that the organization needs in order to make treatment authorization decisions or to meet certain standards required by state or federal governments. They also use the templates to help them meet specific accreditations such as NCQA and JCAHO.

Healthcare organizations require treatment plan forms from their provider panels so that they can give initial or continue giving services to patients. These forms are important as a source of information that will determine the necessity for treatment, show the progress of the patient and show whether clinicians adhere to required treatment plans. The treatment plan template requires that clinicians describe the current problems and symptoms affecting patients, any previous treatments that the patients have received, risk assessment, diagnosis, treatment goals and current medications. In healthcare organizations dealing with substance abuse, the templates require the clinicians to state whether they have tested patients for drug abuse and whether the clinicians have notified the patient’s guardians regarding the treatment given to the patient. The clinician is also required to provide information as the treatment plan continues.

It is essential for clinicians to keep in mind the philosophy or orientation of managed care as they write treatment plans related to treatment of patients with behavioral problems. Healthcare organization managers may not approve plans that include long treatment periods except for those patients with certain illnesses such as Schizophrenia. There are treatment plan template guides that clinicians can follow in drafting good treatment plans. These guides suggest having a treatment plan section and a recommendation section. The treatment plan section includes the goals, interventions and outcomes that a clinician should have in mind when treating a patient. For example, clinicians may have the goals of alleviating symptoms in patients and ensuring that they achieve good results. In the section of interventions, the clinicians will state what they need to do and what they require the patient to do. These statements should be in behavioral terms. In the outcomes section, the clinicians should state in behavioral terms the different actions that the patient will do and the goal attainment criteria.

Treatment plans are important in healthcare organizations because they help clinicians in understanding their patients, their problems and coming up with solutions to the problems. The patients are also able to utilize treatment plans to see their progress in getting well. The people paying for the treatment of the patient also benefit from having access to detailed patient information on the treatment plan.
